Student Health Services has implemented a new procedure to a ssist our students and their families with the submission of the statements to their insurance companies. As you know, Student Health Services does not currently bill students’ insurance plans. The form shown at the bottom of this letter is called a Health Insurance Claim Form 1500 (or HCFA - 1500). This is the required billing form used by most insurance companies to reimburse their members. Due to a change in format of the statements, we are including the example below with highlighted instructions on how to compl ete the necessary areas in the top half of the f or m before mail ing it to your individual insurance company. The bottom half of the form is completed by Student Health Services. You will need your insurance card to fill out the information on every HCFA - 1500 form you receive . To complete, please follow these steps:  Section 1a: o Enter the ID number f r o m your insurance card if you have your own insurance plan. o If you are a dependent on an insurance plan (i.e. child or spouse/partner of insured) enter ID number of primary insured on the plan.  Section 4 : This is the name of the policy holder of the insurance plan. The policy holder is you if you have your own insurance plan or your parent , spouse, or partner if you are a dependent on someone else’s plan. .  Section 7 : This is the address of t he person you have indicated in section 4.  Section 11 : This is th e insurance plan’s group number from your insurance card. Also complete 11 a - d .  Sections 9 through 9 d : Complete only if you are covered und er an additional insurance plan ( secondary insurance ) . “I nsured ” refers to the policyholder of th at insurance plan . Additional information:  If you are covered by a Blue Cross/Anthem/Regence plan, please contact/ call customer service to obtain the correct billing address as it may not be the address on the back of your insurance card.  Please note: the HCFA - 1500 will not work for the purpose of submission to a medical savings account. Please contact the Billing Office at 541 - 737 - 2068 to request a form that will work for that purpose. The time is approaching for you to la unch your educational car eer at what is to become your new educational home , Clark Atlanta University ! As you are preparing for your arrival, I would like to share some helpful information. Enrollment into Clark Atlanta University : The administration, faculty, staff and students want you to become full members of our CAU Family, so please complete your enroll ment process by January 11, 2012. • If you are currently enrolled, we applaud you for completing the enrollment process for the spring 2012. Please sit b ack and enjoy your educatio nal journey during the New Student Orientation. • However, if you have not completed the enrollment process then we strongly encourage you to become enrolled by settling yo ur financial obligations to the institution during the New Student Orientation, January 9 th -10 th , 2012. • Incoming freshmen/transfer students you must have completed the enrollment process and be officially enrolled by January 11th . If you are not officially enrolled by January 11 th , your class schedules/residence hall room assignments will be purged and the institution will not provide you with a meal plan . Arriving to Campus : • All incoming freshmen/transfer students ar e welcome to move into your assigned residential hall starting on Saturday, January 7, 2012 from 12 noon- 9PM . Please report directly to your assigned residential hall where you will receive your keys to your room and a temporary Orientation card for us e as your residential hall door pass key. o Please note students are not permitted to have microwaves or small appliances in your traditional residential hall. Th ere will be periodic room checks by hall Resident Assistants (RA’s) to ensure these items are not being utilized. This is for your safety and all other residents. • Because of limited parking near our re sidential halls, CAU encourages all parents/guardians to park briefly near your residential hall to unload your vehicle and then immediately move to the CAU Parking Deck on the corner of Mildred and Fair Streets. • Panther Paw Office, located in the Bishop Cornelius L. Henderson Student Center, will be open from 10:00am-2:00pm on Saturday, January 7, 2012 , for enrolled students to receive their Panther Paw ID card. Meal Plans for New Student Orie ntation (NSO) Participants: • For all incoming students who have not completed the enrollment process prior to January 9 th : o Meals will not be provided for you on January 7 th and 8 th ; you must be prepared to pay for your meals during th ese dates. For these two days you are welcome to eat in the William Krogman Dining Hall located in the Bishop Cornelius L. Henderson Student Center , lower level for a nominal cost. o The New Student Orientation card will grant you access to meals in the William Krogman Dining Hall during the New Student Orientation dates of January 9 th -10 th only . • For all incoming students who have completed the enrollment process prior to January 9 th : o You must visit the Panther Paw Office between the hours of 10:00am-2:00pm to pick up your Panther Paw card (the official school identification). You will utilize your Panther Paw Card to a ccess your meal plan in the William Krogman Dining Hall located in the Bishop Cornelius L. Henderson Student Center, lower level. New Student Orientation What to Wear: • You are to wear Business Casual (dress pants, skirts, blou ses/polo shirts, dress shoes) attire for all day events on Monday –Tuesday, from 8am-5pm . We are asking that you refrain from wearing jeans, shorts and tennis shoes during the business hours. • You are to wear Casual attire (appropriate jeans with no rips/holes, tops with non- offensive language, and no tank tops fo r men) for eveni ng events after 5pm . On behalf of the School of Music and the jazz faculty at the university, we are delighted that you have chosen to spend a few days with us. You will have lots of opportunity to further your jazz education with an expert group of jazz musicians and teachers. Here are a few items of basic information for all of you. For those of you residing in the dormitory, you may check - in after 1 2:00p m on Sunday at Royer Activities Center at 101 Curl Drive . Students will be staying in Blackburn House, a dormitory located on the North side of campus. Commuting students should arr ive at Weigel Hall at 2:30pm for the Jazz Camper Orientation meeting. All jazz campers are required to attend the Jazz Campers Orient ation meeting on Sunday, June 17 at 2 :30pm in Weigel Hall 174 . Parents are invited but not required to attend this session, which should last about 45 minutes. At that meeting, students will meet the camp faculty and staff and receive a detailed camp schedule. All rules and regulations will be discussed in great detail. At the conclusion of the orientation, we will have pla cement auditions to determine the best music part for you. Remember to bring your instrument with you to the orientation meeting ! For the majority of camp, the student will be participating in activities in Weigel Hall . Weigel is the main building of the School of Music and is located on College Rd. at 17 th Ave., adjacen t to the Wexner Center for the Arts and across the street from the Oval. College Rd. parallels and is one short block west of High St. There are three visitor parking garages within walking distance of Weigel: Arps Garage, Ohio Union South Garage, Ohio U nion North Garage . Hourly parking rates are posted at the entrance of each garage. Credit cards are accepted at the garages. For more information about parking, please visit: rs/parking.php . W e are very excited about your attendance at Jazz Camp. We’ll make lots of music, learn more about jazz, and have a great deal of fun.
