Investigators revealed Monday that "all good here" were among the last words heard from the crew, who all died on board. Two dozen witnesses are scheduled to testify, but the company's engineering director, Tony Nissen, was the first to take the stand. He admitted that he felt rushed to get the vessel ready for the water and described safety concerns that allegedly went ignored. It was struck by lightning during a test mission in 2018, which might have compromised its hull. The Coast Guard confirmed that the Titan suffered a slew of problems during previous trips, including at least 70 equipment issues in 2021 and 48 more in 2022. "There are no words to ease the loss endured by the families impacted by this tragic incident," said U.S. Coast Guard Captain Jason Neubauer, who led the hearing. State officials called her death a "preventable" result of a delay in care because a critical procedure is now illegal — with exceptions that doctors warned were hard to decipher.
